I know the Vampire Lord perks can't be reset, so you might keep them if you cure yourself. I have yet to do a Vampire Lord run myself, so I don't know for sure. I reccommend making a save while you are still a Vampire Lord, then testing it by getting cured and then have Serana turn you back to see if you keep the perks or not.
