Keys are obtained by running a mythic dungeon without a key (at m0 or greater difficulty) or from the Great Vault in Oribos. If you get your first key from a mythic 0 it will be for a +2 dungeon. If you get it from your weekly vault, it will be for a random dungeon at the same level as your highest completed run the previous week.
The fact that there are so many anti-melee affixes, in general, is garbage.
